Hospital Margin Gap Analysis

Methodology version: hospital-margin/v1

Data Source

All financial data in this analysis derives from the CMS Hospital Cost Reporting Information System (HCRIS), specifically the form CMS-2552-10 (Hospital and Hospital Health Care Complex Cost Report). CMS publishes this data annually as a public-use file. It is public domain under 17 U.S.C. § 105.

HCRIS contains self-reported financial and operational data filed by Medicare-participating hospitals. Each hospital submits a cost report covering a 12-month fiscal year. The data includes:

  • Net patient revenue (NPR)
  • Total operating expense (TOE)
  • Total other income
  • Net income
  • Bad debt expense and charity care cost
  • Cost-to-charge ratio
  • Days cash on hand
  • Certified and available beds, total discharges

Scoring Formula

Operating Margin
operating_margin_pct = (NPR − TOE) / NPR × 100

Where NPR = Net Patient Revenue, TOE = Total Operating Expense. NULL when either component is absent in the cost report. Units: percentage points (e.g. −8.5 means −8.5%).

Total Margin
total_margin_pct = (NPR + other_income − TOE) / (NPR + other_income) × 100

Includes non-operating income. Denominator guard: NULL when (NPR + other_income) = 0.

National Average
national_avg = mean(operating_margin_pct) across all hospitals with non-null operating_margin_pct in the issuance period

Snapshot at compute time. Stored in each row for stable historical gap comparisons.

Margin Gap
margin_gap_pct = operating_margin_pct − national_operating_margin_avg

Negative = below national average. NULL when either operand is NULL.

Financial Distress Threshold

A hospital is flagged as financially distressed when its operating margin is below -5% (operating_margin_pct < -5).

This threshold is consistent with the financial distress signal used by Moody's Investors Service, Fitch IBRS, and the Medicare Payment Advisory Commission (MedPAC) in hospital credit-rating analysis. A hospital operating at −5% or worse for a sustained period typically triggers a credit watch or covenant review under standard bond indenture agreements.

The −5% threshold is pinned to methodology version hospital-margin/v1. Any change to this threshold requires a new methodology version and a new wave.

Fiscal Year Conventions

HCRIS allows hospitals to use non-calendar fiscal years. The issuance_period in this analysis is derived from the most common fiscal_year_end year among hospitals in the dataset. Hospitals with different fiscal years are included in the same issuance period by their fiscal year end year.

CMS typically publishes the final annual HCRIS file in the fall (October–November) for the prior fiscal year. The annual cron (December 1) triggers recomputation after the operator runs the HCRIS ingest script.

Limitations

  • Self-reported data. HCRIS figures are filed by hospitals and subject to CMS audit; material restatements occur.Fonteum does not independently audit or validate hospital cost report filings.
  • Coverage. Only Medicare-participating hospitals that file CMS-2552-10 are included. Critical Access Hospitals (CAH) file form 2552-96 and are currently excluded from this analysis.
  • Timing. HCRIS data lags 12–18 months behind the fiscal year end. This analysis reflects conditions from the most recent available annual release, not the current fiscal year.
  • Hospital identity. Hospital name and state enrichment from HCRIS header fields is a follow-on wave; CCN is always present. Name-to-CCN joins are available via NPPES and CMS Care Compare.
  • Single-entity only. Multi-entity health systems file consolidated cost reports that may obscure facility-level performance. Facility-level data reflects what CMS receives, not necessarily the consolidated entity.
